How Do Patients qualify?
To qualify for service, patients require physician certification and must meet the appropriate criteria for admission. Treatment can be initiated through referrals from a hospital, physician's office, rehab facility (inpatient), long term care facility, managed care, personal care facility, and other third party payers.
What is the cost to patients?
Traditional Medicare (Part A) pays 100% for home health services. If you have another type of insurance, you can inquire with our referral department about coverage of services.

We will provide complete collaboration that puts the person first and that the entire care team is focused on continuous feedback and innovation of both the technical documentation as well as the therapeutic therapies and care delivery provided to each patient. Each patient will have the opportunity to participate in developing their own care plan so that they have complete choice of how and what the care team is delivering to ensure each patient is receiving what they want. We will drive more loyalty from our care team, which in turn provides better staffing for our patients, by providing them with the latest tools and having an extremely responsive backend team dedicated to help them with any issue that may arise.
